Three weeks ago i awoke to find i had severe neck pain in the left side of my neck. It hurts to yawn, stretch, and turn especially when i am sleeping. I now feel pain down into my shoulder. The pain is always there and although ibuprofen helps ease it, the pain is still there. I would rather not go to the doctor, but any advice would be gratefully received. I am 35 year old female.<br />

My head is tilted to the left because I can&#8217;t seem to get it straight, and my shoulders and tilited along with it. Every time I move it gets worse and hurts so bad I start crying. What do I do? Btw, I&#8217;m 12!</p>

<p><i>Answer by gillianprowe</i><br/>Rest it until the pain subsides and then start exercises, look all the way to the right and back to the centre and then all the way to the left and back to centre. Pain has a way of making us tighten up all those muscles, so you need to relax. Try and get someone to give you a relaxing neck massage and try standing in a warm/hot shower and allow the water to beat off the painful part of your neck.</p>

<p> Neck pain is a very common problem affecting up to 70% of the adult population at some point in life. Though there are specific causes of neck pain such as arising from a sports injury, a car accident or &#8220;sleeping crooked,&#8221; as Dr Barry Hitchcock explains, the vast majority of the time, no direct cause can be identified and thus the term nonspecific is applied. There are many symptoms associated with patients complaining of neck pain and many of these symptoms can be confused with other conditions. Wouldn&#8217;t it be nice to know what neck related symptoms are most likely to respond to chiropractic manipulation before the treatment has started? This issue has been investigated with very favorable results!</p>
<p> The ability to predict a favorable response to treatment has been termed, &#8220;clinical prediction rules&#8221; which in general, are usually made up of combinations of things the patient says and findings from exams. In a large study, data from about 20,000 patients receiving about 29,000 treatments, was collected and analyzed to find out what complaints responded well to chiropractic treatment. The results showed that the presence of any 4 of these 7 presenting complaints predicted an immediate improvement in 70-95% of the patients: 1. Neck pain; 2. Shoulder, arm pain; 3. Reduced neck, shoulder, arm movement; 4. Stiffness; 5. Headache; 6. Upper, mid back pain, and 7. None or one presenting symptom. Items not associated with a favorable immediate response included &#8220;numbness, tingling upper limbs,&#8221; and &#8220;fainting, dizziness and light-headedness in 4-12% of the patients. The &#8220;take-home&#8221; message here is that was far more common to see a favorable response (70-95%) of the patients compared to an unfavorable response (4-12%), supporting the observation that most patients with neck complaints will respond favorably to chiropractic treatment.</p>
<p> So, what do we do as chiropractors when a patient presents with neck pain? First, after gathering preliminary information such as name, address and insurance information, a history of the presenting complaint is taken. This consists of information including what started the neck complaint (if you know), when it started, what makes it worse, what makes it better, the quality of pain (aches, stiff, numb, etc.), the location and if there is radiating complaints, the severity (0-10 pain scale), timing (such as worse in the morning, evening, etc.), and if there have been prior episodes. Various questionnaires are included that are scored so improvement down the road can be tracked and a past history that includes a medication list, past injuries or illnesses, family history and a systems review are standard. The exam includes vital signs (BP, pulse, height, weight, temperature and respiration), palpation, range of motion, orthopedic and neurological examination. X-ray and/or other &#8220;special tests&#8221; may also be included, when needed. A review of all the findings is discussed and after permission to treat is granted, a chiropractic adjustment may then be rendered. A list treatment options may include: </p>
<p>1. Adjustments; 2. Soft tissue therapy (trigger point stimulation, myofascial release); 3. Physical therapy modalities; 4. Posture correction exercises and other exercises/home self-administered therapies; 5. Education about job modifications; 6. Co-management with other health care providers if/when needed. <p>More than eighty percent of all people will suffer from low back pain in their lifetime. The area that is most effected by pain is the lumbar spine which happens to carry the most weight due to it&#8217;s bio-mechanical position against gravity. The most common instances of low back pain can be caused by poor lifting techniques, obesity, and poor postural positioning and ergonomics.</p>
<p>It has been said that the majority of lower back pain resolves itself within fourteen days, based on a study that was done ages ago. What this study does not mention are the factors that were not taken into account, showing a skewed proportion since many people did not finish the study. These people were then considered cured of their symptoms, which is most likely false. Yet, these finding set the standard for back pain treatment in North America.</p>
<p>The chiropractic adjustment continues to be the most vital thing a chiropractor can provide to help the body heal itself. The symptoms of low back pain, neck pain, and headaches can be included in what these adjustments are used for. The spine is analyzed a series of specific pressures are applied to increase range of motion and remove spinal pressure and increase alighment of the spinal column itself.</p>
<p>A form of therapy called, spinal manipulation, is the application of controlled force to a joint, moving it beyond the normal range of motion. It is used by chiropractors to reduce pain, increase mobility, and restore biomechanical and anatomical relations between spinal segments to facilitate rapid recovery. Spinal manipulation therapy can prevent the onset of a chronic disorder associated with immobility which results in successful recovery especially if combined with exercise and good body mechanics. Chiropractic treatment takes approximately 3-6 weeks to treat patients with nonspecific and uncomplicated low back pain and up to 10 weeks for those with more severe cases.</p>
<p>The outcomes of clinical research done on chiropractic patients showed that the people who received treatment from a local chiropractor were more satisfied than the group who was treated with western medicine. </p>
<p>According to a survey, chiropractic treatment received the highest percentage of patients having 59% who were highly satisfied compared to any other treatments, including physical therapy (55%), acupuncture (53%), massage (48%) and treatments from specialists (44%) and primary-care doctors (34%). It was found that hands-on treatments are very helpful with the patients suffering from low back pain.</p>
<p>Low back pain may be common, but each patient has their unique experience with their pain. It is appropriate to use biopsychosocial measures to individualize treatment than to use a &#8220;one size fits all&#8221; approach. Multiple efficacious treatments also introduce greater opportunity for choice which may bring a positive outcome for a patient&#8217;s treatment.</p>
